Preventive healthcare has become a central EU policy priority, driven by rising chronic disease burdens, demographic ageing, widening health inequities, and growing pressure on health workforces. Digital transformation creates new possibilities for proactive, personalised, and continuous prevention at scale. Yet, realising this potential depends on systems that are interoperable, governed, and trusted. This study assesses the technological feasibility, system readiness, and long-term sustainability of establishing a European-level digital platform to support preventive healthcare. Designed as a citizen-facing, AI-supported environment, the platform would provide personalised prevention guidance, integrate health and lifestyle data from multiple sources, and complement national pathways for preventive healthcare while leveraging the EU's advancing digital health architecture. The study evaluates technical and infrastructural prerequisites, assesses Member State readiness, and analyses the opportunities and risks of AI deployment for preventive healthcare.
